Shall this measure to fund and maintain police, fire, and paramedic costs by imposing a public safety special tax from July 1, 2025, to July 1, 2032, to raise approximately $2,037,000 yearly, with annual audits and citizen oversight provided, at the following rates (subject to increases in CPI and certain exclusions): - Single-family residential parcels - $480 per parcel per year,-Multi-family residential parcels - $576 per parcel per year, and - Non-residential parcels - $660 per parcel per year; be adopted?

Shall City Council Ordinance No. 1461 approving a zoning map amendment, a specific plan, and a development agreement for The Meadows at Bailey Canyon project located at 700 North Sunnyside Avenue in Sierra Madre be adopted?

Shall the general plan land use designation change from "Institutional" to "Hillside" and the zoning designation change from "Institutional" to "Hillside Management" for the Mater Dolorosa Property?

Shall the office of the City Clerk be appointive?

To maintain police, fire and paramedic response times; maintain police patrols, emergency planning and response; provide for parks, streets, sidewalks and parkway trees, library services, recreation programs, senior programs, and general finances, shall the City of Sierra Madre establish a 3/4¢ sales tax until ended by voters, providing approximately $225,000 annually with all revenue remaining in Sierra Madre for the benefit of its residents?

(ADVISORY) If Measure "D" passes, repealing the Utility Users' Tax and eliminating approximately 24% or $2.6 million of the City's General Fund Revenues, should the City Council eliminate paramedic services, reduce and outsource police services and library services, reduce code enforcement, and fire suppression services, in addition to other reductions which will be required to balance the budget?

Shall the City of Sierra Madre adopt a measure repealing the City's Utility Users Tax in its entirety?

To maintain City services, such as Public Safety, Paramedic and Library services, shall an Ordinance be adopted to: 1) Increase the City's Utility Users' Tax (UUT) to a maximum of 10% for all utilities, to be effective on July 1, 2016; and 2) Extend and stabilize the funding by eliminating the "sunset" clause; and 3) Provide an exemption from the UUT for very low income households?

To maintain City services, such as Public Safety, Paramedic and Library services, shall an Ordinance be adopted to: 1. Extend the City's current 10% Utility User Tax at a maximum of 10% for all utilities except water and sewer; 2. Establish new sunset dates starting July 1, 2022; 3. Extend the current 9% water and sewer tax until July 1, 2014, when it will reduce to 7%, and then to 6% on July 1, 2015?

Shall an Ordinance be adopted to amend Sierra Madre Municipal Code Section 17.35.040 (“Core Density Limit”) of the People’s Empowerment Act (aka Measure V) to permit development of an assisted living facility consistent with the Kensington Assisted Living Facility Specific Plan not exceeding two stories, thirty feet in height and seventy-five assisted living suites, for the parcels located at 33 North Hermosa Avenue an 245 West Sierra Madre Boulevard?

Shall an Ordinance be adopted amending the City's existing utility Users' Tax to continue the existing 10% tax, subject to potential increase of 12% on July 1, 2013, in otder to maintain general City services such as public safety services, including paramedic programs, and to reflect technological advances in communications, continue existing exemptions to low and very low income households, establish new sunset dates and continue a citizen's oversight committee?

(ADVISORY) If Measure 12-1 to continue the existing Utility Users' Tax of 10%, subject to a potential increase to 12% in July 1, 2013 and establishing new sunset dates, is approved by the voters, should the additional revenue generated by that tax be used to fund public safety services, including paramedic programs?
